There is the contact form plugin we recommend:

http://wordpress.org/extend/plugins/contact-form-7/

Once you upload it to your plugins folder, activate it from the plugins tab, you'll need to insert one line of code (in the readme and instructions), set some basic configurations and you're good to go.

I've used these contact forms for over a year now and they are amazing. It's the easiest way to allow your prospective clients to get in touch with you.

I just installed CFORMSII and it is great.  I tried the green-beast.com form because it seemed very popular, but was harder to use and too manual when it came to coding.

I also use "Secure and Accessible PHP Contact Form v.2.0WP" on several sites. Its easy to configure, and seems robust to the point of being bullet-proof.

Pros:
Users like the "Carbon Copy" option
The anti-spam filter is simple but effective
Works with multiple versions of Wordpress

Cons:
No option to add custom fields... that would be good :-)

I am using cformsII, it has easily customizable fields.  The con I have is that it says "delicious days" at the very bottum.

you can remove the delicious days credit - not sure how but my tech did it
